alhamdulillah,exam psy dah berlangsung semalam.lega rasenye lps exam!lps ni blh pg jln2…lame x kuar jln2..(tp xde la lame sgt kot…x sampai lg sbln;P)
posting psychiatric byk buat ak berfikir pasal kehidupan org lain…sometimes, im wondering how they can get the illness esp schizo…camne die blh nampak mende pelik2, dgr suara yg bukan2…btl ke die nmpk sume tu,ape yg diorg rs time tu…erm,even ade hypothesis yg kate ade neutransmitter dysregulation tp hypothesis ni tak semestinya 100% correct..(e.g: dlm schizo ade increase dopamine, NE, and serotonin ; reduce GABA)…knp neurotransmitter ni blh increase ek..erm, mmg ade certain substance yg blh increase these neurotransmitter…for example alcohol.pathophisiology–>[Alcohol-related psychosis most likely
relates to dopamine in the limbic and possibly other systems. The
dopamine hypothesis often is applied to psychosis involving excessive
activity of the dopaminergic system. Animal studies have shown
dopaminergic activity to increase with increased release of dopamine
when alcohol is administered. On the other hand, alcohol withdrawal
generates a decrease in the firing of dopaminergic neurons in the
ventral tegmental area and a decrease in release of dopamine from the
neuron.]..so, alcohol drinker can has psychotic symptom such as hallucination(esp visua hallucination)…tp ade gak patient yg xde family history of mental illness, xde substance abuse tp dpt schizo..knp ek?huhu..mmg sometimes dpt cari diorg punye predisposing factor but there is no clear link between the factor and the illness…kesian gak tgk psy patient esp schizo patient sbb diorg biasenye has poor insight…diorg x th pun diorg sakit…huhu..erm,mungkin ade hikmahnye di sebalik ape yg berlaku kat diorg yg kite x th:)
erm, dlm exam psy ni ak dpt case Bipolar 1 disorder in manic phase and the examiner was DR. Norjan(quite nice lecturer:))..patient ni ade thyroid lump..so dptla bg differential diagnosis mania secondary to hyperthyroidism. provisional diagnosis still Bipolar 1 disorder in manic phase sbb die ade psy illness for past 16 years. lump tu br lg ade, pastu die xde hyperthyroid symptom.dr norjan byk tye pasal psychopathology..antara soalan yg die tye what is flight of idea, connection in FOI, (Dr x puas hati dgn jawapan yg ak bg utk clang association..huhu), spending spree, component of elated mood and management utk patient tu including side effect of lithium, maintainence level of lithium,what condition can lead to lithium toxicity. how to monitor patient on lithium and sume pasal lithium..erm, pastu bile ak start nk bgth psychosocial management loceng pun bunyi..times up!, baru je sebut word psychoeducation, x sempat pun nk bg explaination and sebut managent yg lain…ak nk sambung jwpn plak pastu tp ade next candidate plak nk masuk kat examination room tu..Dr. ckp"ur time is finished, u can go now"..huhu..terpinga2 aku kat situ;) terkejut sbb exam dah habis without finishing my answer..sekejap je mase 30 min berlalu…ak x th DR bg markah berape kat aku..aku harap aku lulus dgn cemerlang,insyaAllah.alhamdulillah, patient yg ak dpt co-operative even she was very2 talkative but still manageable…mase clerking ak kelam kabut gak sbb ak hanye ade mase 1 hour, pastu patient asyik crite ape yg die nk crite..pastu about 40 minutes of clerking patient tu ckp die nk pg minum, dah masuk waktu mkn, die nk kuar dr bilik clerking tu..huhu..nasib baik dpt pujuk die..ak x siap lg clerk die..erm,exacerbate ak punye anxiety plak..huhu.nasib baik die x kuar dr bilik tu.
utk teori , MEQ pasal schizo, panic with agoraphobia and dementia.sume soalan clinical scenario..it was asking about diagnoses for the patient, other question that need to ask to support the diagnosis, prognostic factor, investigation and management. soalan MCQ was quite ok. tp ade gak soalan aku x jwb sbb x yakin and x th nk jawab..hehe..nnt klu salah jwb kene minus marking.;)
alhamdulillah semunye berjalan dgn baik..skg tgh palpitation tunggu result..hopefully, i’ll pass in this exam.insyaAllah.hanya kepadaMu ya Allah aku berserah.ameen.
